The Voter Registration Form (EL-131) must be completed accurately and completely. The form should be mailed to: City Clerk, 2000 N. Calhoun Road, Brookfield, WI 53005 before the "registration by mail" deadline, (3rd Wednesday prior to Election Day). If you have moved from the City of Brookfield to another municipality, you may fill out the Authorization to Cancel Registration (PDF) Opens a New Window. form. Upon receiving the form, the Clerk’s office will then cancel your voter registration. Wisconsin law requires you to have lived at the residence at least 28 days prior to registration and requires acceptable proof of residency. Application for Absentee Ballot If you are not already registered, you will need to register to vote before an absentee ballot can be sent to you. All requests for an Absentee Ballot must be in writing by the elector. Photo ID is required with the application. The application must be received by the clerk no later than 5:00 PM on the Thursday prior to Election Day in order for an absentee ballot to be mailed to you. Request in Person You can request an Application for Absentee Ballot in person at the City Clerk’s Office. This can be done beginning the Tuesday 14 days prior to Election Day and ending at 5:00 PM on the Friday before Election Day. Deadline for Returning Ballot The completed ballot must be received by Election Day for delivery to the Central Count location.
